You can also pick up donation forms at the Gala Auction Donation Table in the Courtyard on Sundays after service.

Please complete this form legibly, attach it to your donation item, then drop off at: Spiritual Life Center Courtyard Auction Table after Sunday Services at 2201 Park Towne Circle, Sacramento, CA 95825. Donation items can be dropped off on Sundays.
